When this form is submitted, it generates a Google Form for attendance (and a corresponding Google Sheets file) and makes a short URL that points to it. If you submit this form multiple times using the same club name, previously generated Google Forms for that club will be trashed, but the same spreadsheet will be used. Do not change the name of the spreadsheet, or this will not work. You can, however, move the spreadsheet into a folder. This will only work if you're signed into a WRDSB account.
To use this, type in the club name at the start of every meeting and press submit. Share the short URL with club members in-person and have them input their names in the form. A spreadsheet will be created in your drive that is automatically updated.
